Support Coordination
Job Overview
We are seeking a compassionate and detail-oriented Support Coordinator to join our team. In this role, you will assist individuals with developmental disabilities in gaining access to essential services, resources, and support. You will ensure that each person’s unique needs are met through person-centered planning, coordinated care, advocacy, and continuous monitoring of service delivery. As a Support Coordinator, you will serve as the primary point of contact between the individual, their family or guardian, service providers, and community agencies, helping to promote independence, inclusion, and an improved quality of life.
Responsibilities
Develop, implement, and monitor Individualized Service Plans (ISPs) in collaboration with the individual, family, and service team.
Conduct regular home and community visits to assess the individual’s well-being and ensure services are being delivered effectively.
Coordinate access to medical, behavioral, educational, vocational, and community-based services according to the individual’s goals and needs.
Advocate on behalf of individuals to ensure their rights, preferences, and choices are respected.
Maintain accurate and up-to-date case records, progress notes, and documentation in compliance with state and agency standards.
Facilitate interdisciplinary team meetings and ensure ongoing communication among all parties involved in the individual’s care.
Identify and address service gaps, barriers, or issues affecting the individual’s quality of life.
Assist individuals and families in understanding available resources, eligibility requirements, and how to access support.
Stay informed on state regulations, community resources, and best practices related to support coordination and disability services.
Participate in staff meetings, training sessions, and professional development activities as required.
Skills
Bachelor’s degree ( preferred in social work, Psychology, Human Services, or a related field).
Experience working with individuals with developmental disabilities or similar populations.
Strong communication, organizational, and problem-solving skills.
Ability to work independently and manage multiple cases efficiently.
Knowledge of community resources, Medicaid, and state service systems.
Proficiency in Microsoft Office and electronic documentation systems.
Must possess a valid driver’s license and reliable transportation.
