Position Title: Social & Community Service Coordinator
Key Responsibilities:
Develop and implement strategies to improve community health outcomes.
Coordinate programs and materials addressing health issues affecting diverse populations.
Collect, analyze, and report data to identify health-related concerns and measure program effectiveness.
Evaluate and adjust existing programs to better meet community needs.
Provide education on available healthcare services and serve as a link between the community and healthcare professionals.
Maintain records in compliance with HIPAA, State, and Federal regulations.
Offer informal counseling, social support, and assistance with accessing resources.
Prepare and submit monthly reports, including program evaluations and outreach strategies.
Assist with securing funding and resources to sustain programs and services.
Minimum Requirements:
Bachelor’s degree in Public Health, Social Work, Psychology, Human Services, Mental Health, or a closely related field.
At least 6 months of relevant job experience.
Strong organizational, communication, and community engagement skills.
Ability to work collaboratively with both internal staff and external partners.
