Position Title: Social & Community Service Coordinator

Key Responsibilities:

  • Develop and implement strategies to improve community health outcomes.

  • Coordinate programs and materials addressing health issues affecting diverse populations.

  • Collect, analyze, and report data to identify health-related concerns and measure program effectiveness.

  • Evaluate and adjust existing programs to better meet community needs.

  • Provide education on available healthcare services and serve as a link between the community and healthcare professionals.

  • Maintain records in compliance with HIPAA, State, and Federal regulations.

  • Offer informal counseling, social support, and assistance with accessing resources.

  • Prepare and submit monthly reports, including program evaluations and outreach strategies.

  • Assist with securing funding and resources to sustain programs and services.

Minimum Requirements:

  • Bachelor’s degree in Public Health, Social Work, Psychology, Human Services, Mental Health, or a closely related field.

  • At least 6 months of relevant job experience.

  • Strong organizational, communication, and community engagement skills.

  • Ability to work collaboratively with both internal staff and external partners.
